Use Form Manager to create contact forms with different field types and store them in the site database.

To insert an existing form, go to the INSERTING FORMS section to learn how to do it with the SMBCMS Editor
To create a new contact form, click Add New Form. Form Wizard window opens.
To delete an existing form, check the Delete check-box near its name and click Save.

Enter the required information in the Form Wizard window.

E-mail – an e-mail address the form will send messages to.
Name of form button – a button caption (e.g., Send, Submit, etc.).
Number of Fields – set a number of fields for the contact form.

To continue, click Next Step.
To close this window, click Cancel.

Field caption – enter a caption for the field.
Field type – set field type. This feature allows SMBCMS to validate information provided. If information entered by a visitor is incorrect (e. g., a text instead of a number), he won’t be able to send this form.
Required – checking this check box makes the field required for visitors to fill in.

To continue, click Next Step.
To return to the previous step, click Previous Step.
To close this window, click Cancel.
